Using Your Health Insurance

Student Health Services has begun billing most major health insurance companies as an in-network provider. We courtesy bill all health insurances except out-of-state Medicaid and Medicare.

Verifying Your Insurance Coverage

Out-of-pockets expenses depend on each student's individual health insurance plan. Contact your health insurer to better understand your coverage and whether you receive in-network or out-of-network benefits. When contacting your insurance company please use our Tax ID and/or NPI number, not individual provider name, to ensure we are in network. Oregon State University bills as an entity and not by individual provider.

NPI: 1316048531
Tax ID: 61-1730890
Address: 850 SW 26th St., Corvallis, OR 97331

In-Network Insurers

Student Health Services bills most major health insurance companies as an in-network provider. Please contact your insurance plan to verify coverage, especially if you have a PPO, HMO, ECO, MCO, HDHP, or any Kaiser health plan. Your plan may require you to be seen by certain health care providers.

Government Plans

All government plans are accepted except out-of-state Medicaid and Medicare. OSU Student Health is actively enrolled in the Oregon Health Plan (OHP).

We recommend adequate health insurance coverage for all Oregon State students. Health insurance provides financial protection against unexpected medical expenses and facilitates access to services, such as preventive screenings, vaccinations, and mental health support.

Don't have health insurance? Oregon State University offers the Student Health Insurance Plan (SHIP) — an affordable insurance option that is easy to use at our clinics. International students are required to enroll in SHIP and are automatically enrolled.

You may also be eligible for Oregon Medicaid. To learn more and find out how to enroll, contact a Benton County Health Department health navigator. The coordinated care organization affiliated with Student Health Services is OHP-IHN.

Before your First Appointment

  • If you have insurance, contact your health insurer to verify your coverage. This is an important step if you have in-network or out-of-network coverage to better understand how your benefits will work in our clinics. Typically, there is a member services phone number on the back of your health insurance card. Your insurer can look up Oregon State University by our NPI (1316048531) or tax ID (61-1730890).
  • Prior to your first appointment, visit the patient portal. If you have insurance, submit your insurance card details in the portal. It is a student’s responsibility to make sure Student Health Services has the most current health insurance information at each visit.
  • As applicable, have ready the insurance subscriber's name, address and date of birth for the appointment. Note that the subscriber could be a parent, spouse, domestic partner, or guardian.

Common Questions About Insurance

Health insurance pays part of the cost of health care for you and your family if you are injured or get sick.

Beginning the 2025-26 academic year, Student Health Services will bill most major health insurance companies as an in-network provider. We still courtesy bill all health insurances, other than out of state Medicaid and Medicare, which we are unable to accept. Students should contact their insurance company to find out whether it is in network at Oregon State and to learn about coverage. Provider your insurer with OSU's NPI (1316048531) or tax ID (61-1730890).

Some questions you may want to ask your insurer include:

  • Do I receive in-network coverage at Oregon State University?
  • If I do not have in-network benefits, what will my cost be?
  • Does my plan provide coverage for any planned services? Are there limitations or exclusions to my coverage?
  • Does my plan require a prior authorization or referral, or does it have other limitations?
  • Do I need to pay a deductible? If so, how much?

Yes, OHP is accepted for billing at our clinics.

We courtesy bill all health insurances, other than out-of-state Medicaid, which we are unable to accept. Note that insurers may take up to 60 days to respond. If you do not have health insurance, prefer not to provide insurer information, or if your insurance does not cover your charges, your bill will be sent to your student account. We do not collect any payments at the time of your visit.

The billing staff work remotely and would be happy to answer any of your insurance-related questions. Please call 541-737-2068 or message the billing department through the Patient Portal
