Welcome to the OSU Medical Records for Active Duty, Veterans, and family members website. Thank you for your service. This website has been put together to help you navigate through the many different ways of obtaining copies of your medical records, and provide other medical resources for you. Unfortunately, when Veterans get out of the service medical records are not always automatically sent to the VA Medical System, and copies are often not provided to the members. Please take a look at the below information that will help you in requesting your medical records from the services. Once again thank you for your service.

At OSU Student Health Services, you as an Active Duty, Veteran or Family member have access to treatment. You pay a health fee, which allows you to see a clinician. If the clinician decides to do labs, x-rays (now available through Samaritan Athletic Medicine Center), or medications, etc., then your insurance will be billed or the charges may be placed on your student account.

My name is Brian Cooke. I served in the US Navy as a Hospital Corpsman for 9 years (1998-2005). While in the Navy I did various duties within a medical setting. I look forward to providing support to Active Duty, Veterans, and Family members. I am here for you, and will help navigate the different ways of requesting medical record information to help you as an OSU Student to be successful.

National Archives

Information will need to be completed by the Veteran or Service Member. There are questions that only they would know how to answer. Please ask that records be sent to you and forward the information pertinent to SHS (i.e. immunizations).

 

TRICARE Online Patient Portal

This site provides access for eligible members offering online military hospital or clinical appointments, prescription requests, and personal health data.

While this is the general VA website, you can still find VA Hospitals and Clinic information. It also provides links to requesting medical records.

MyHealthy Vet

This website provides access through the VA for those who are enrolled in the VA Health Care System. If you do not have access to this, consider signing up. It gives you access to prescriptions, appointments, complete medical records, and the ability to send messages to your VA clinicians.
