Safeguarding the health of students and the community.
Immunizations offer safe, effective protection from vaccine-preventable diseases. This is especially true for students living in group housing on or off campus.
By reducing cases and outbreaks of significant infectious diseases, Oregon State University’s required immunizations help students stay in class and achieve their academic goals.

Helpful Information on Immunizations
Immunization records should be submitted via the Patient Portal.

If you have not been immunized but are immune based on blood tests or having had the diseases, you must submit official documentation from your medical provider (personal physician, pediatrician or county health department) that you had the respective disease(s) or blood test results
Requesting records
If you need to request records from a doctor or school, feel free to use this Authorization Form to do so.
You may complete this form and submit it directly to your health care provider, which will give them permission to send the information to Student Health Services directly.
Providing proof of immunity
If you are unable to provide proof of immunity to any or all of these diseases, you will be required to have either a blood test to measure your immunity (called a titer), and/or have the immunizations.
Student Health Services can perform blood titers and administer the immunizations. Since titers can be very expensive and they may demonstrate your need to be immunized anyway, it is recommended that you simply take the immunizations.
It is safe to have the immunizations even if you are already immune.
Consequences
A hold will be placed on the registration process of non-compliant students attempting to register for their second term at OSU.
High-risk countries
Students from countries identified as high TB risk are required to complete a QuantiFERON Gold TB test TB upon arrival at Oregon State.
Exempt countries
Students from exempt countries who have lived in a high-risk country for more than six (6) months are recommended to complete a TB test as well.
How the TB testing works
The TB test will be available through OSU Student Health Services.
In many cases, TB testing will be included in the new student orientation schedules as coordinated by INTO-OSU and OSU International Programs.
The testing must be completed within the United States, so please do not pursue advance testing in your country prior to your arrival.
If you are from one of the high-risk countries, please expect to complete your TB test upon arrival at OSU.
Medical exemptions
Immunization requirements may be waived for students with qualifying medical conditions such as pregnancy, diseases which lower resistance to infection, or use of certain medications.
Non-medical exemptions
Non-medical exemptions may be given for those with beliefs, practices or ethical values which prohibit immunizations. Be aware that you may be removed from classes if an outbreak occurs and you are not vaccinated against the disease.

Student Immunization Requirements
Domestic & International Required vaccinations:

Measles, Mumps and Rubella (MMR)
- Entering domestic students born on or after January 1, 1957, must have:

Two doses of measles vaccine given at least 30 days apart on or after the first birthday or physician documentation of measles disease or a lab test documenting immunity to measles disease.

Two doses of mumps vaccine given at least 30 days apart on or after the first birthday or physician documentation of mumps disease or a lab test documenting immunity to mumps disease.

One dose of rubella vaccine given on or after the first birthday or a lab test documenting immunity to rubella disease.

Meningococcal MCV4 - 1 Dose
- Students under the age of 22 must provide documentation of one dose of meningococcal quadrivalent (MCV4) received on or after turning 16 years old.

Varicella - 2 Doses
- Varicella (chickenpox) immunization is required for all students. Date of disease is also accepted.

Tetanus-Diphtheria-Pertussis (Tdap) - 1 dose
- Immunization for tetanus-diphtheria-pertussis (Tdap) dose received within the last ten years.

Hepatitis B - 3 doses

Hepatitis B is required for all students. Date of disease is also accepted.

Domestic & International Recommended Vaccinations:
While not required at this time, we highly recommended the following:

COVID-19 vaccine

Effective June 16, 2023, Oregon State University will no longer require proof of COVID-19 vaccination.

Meningococcal B
- Student age 25 and under are recommended to provide documentation of 2 doses of Bexsero or 2 doses of Trumenba (3 doses recommended if at an increased risk).

Official documentation is required for all vaccines.

Domestic & International Vaccination Deadlines
Domestic students - have until the middle of their first term to receive and submit documentation of their required vaccines. If we fail to receive your documentation in time, a hold will be placed on your account preventing you from registering for your next term courses.
International students - All vaccinations must be completed before admittance to OSU, otherwise registration holds will immediately be placed on your account.
TB testing requirements
Students from countries identified as high TB risk or who have answered "yes" to any of the questions on the TB questionnaire must complete TB testing upon arrival at Oregon State.
This testing is available through Student Health Services. In many cases, testing will be included in your new student orientation coordinated by INTO-OSU and OSU International Programs.
Testing must be completed within the United States, so please do not pursue advance testing in your country. Instead, expect to complete your TB testing upon arrival at OSU.
Countries of High Tuberculosis Incidence
Students from any of the countries listed below are required to complete TB screening upon arrival at Oregon State. Domestic students who have lived in one of these countries for more than six (6) months are not required to be screened, but it is recommended.
