OSU will be hosting Flu Shot Clinics in Corvallis in October for employees and students.  The process will look different this year to comply with COVID-19 requirements.

Dates: October 1 and October 14

Location: MU Ballroom

 

Complete the online registration process located at https://beav.es/flu-clinic.  The online registration process collects information ahead of time and enables you to schedule appointments. There are three steps to this process in order to keep confidential information secure. It is important that you complete all three steps in order to successfully schedule an appointment. 

 

  1. Go to https://beav.es/flu-clinic to complete the initial form with your information.

     

  2. Upon completion, your browser will be redirected to a second form to gather your insurance information.

     

  3. Finally, your browser will be redirected to schedule your appointment.  Use zip code 97331 to access the OSU Flu Shot Clinics. 

     

On the day of the clinic: 

  • Bring your insurance card, photo ID and completed Vaccine Administration Record (VAR) form: Download the form here.
  • Wear a mask.
  • Arrive at your scheduled time.  
  • Follow social distancing protocols and directions.  
  • Wear short sleeves.

 

 

Coverage for the vaccination: 

  • Fred Meyer/Kroger will be billing insurance only and will not accept cash payments this year. 
  • PEBB Statewide (Providence), Providence Choice, Moda and PacificSource plans will cover the cost of the flu vaccinations.
  • Employees/Students with Kaiser Insurance: Kaiser confirmed that the vaccination will not be covered if received at an OSU Flu Shot Clinic. However, Kaiser Permanente offers no-cost flu shots to members at their facilities.  To find a flu shot clinic date and location with Kaiser go to kp.org/flu.
  • Employees/Students with other insurance coverage: Please verify with your insurance provider if the flu vaccination is covered before scheduling at the clinic.

 Yes, the egg free vaccine will be available this year. It is called Flublok. PacificSource, Providence and Moda have confirmed that they will cover this vaccine. Please verify with your insurance regarding coverage if you have other insurance plans.

Yes, dependents may attend the OSU Flu Shot Clinics. Children must be age 7 and older to receive the flu vaccination at these clinics.
