A high rate of vaccination among our students, faculty and staff supports the safety and well-being of our community and the communities in which we operate, especially as we engage in in-person learning, research, and outreach.
PLEASE NOTE: SHS is offering the Moderna bivalent booster for students by appointment.
Vaccine access: OSU will continue to support the regional distribution of COVID-19 vaccines in Corvallis, Bend, Newport and other locations throughout the state.
OSU requires all employees and students who work, learn or engage with others in-person as part of their job duties to show proof of initial COVID-19 vaccinations, though exceptions are allowed when based on medical or disability reasons or sincerely held religious beliefs.
Please upload your proof of vaccination to Student Health Services’ secure Patient Portal.
Once you have your vaccine card, please click here to access the Patient Portal, look for the following button on the front page and enter your information.
You will need to upload your COVID vaccination card. To do this, please follow the following steps.
PLEASE NOTE: Your vaccination will be verified by the immunizations staff manually once both the card upload and vaccination history sections are complete. This is a manual process, please be patient.
Upload a copy of your COVID vaccination card. To upload a copy of your card, click the “Enter my COVID-19 Vaccination Information” button, then click the green “Upload” button. Once completed, you will see an icon-sized version of your card in that section.
Under the “COVID-19 Vaccination History” section, click “Add Immunization”, and add an event with the information from each COVID-19 vaccination dose. Please note: if you get a booster shot you will need to return to this section to add that event to the list.

Your healthcare provider must fill out and certify your exception request.
Sign the form.

To claim a religious exception, complete the EOA Form for a religious accommodation. EOA will review the form and email you back with an answer as soon as possible.
