Note to Incoming Students for Fall Term 2012

New student data is loaded into the Patient Portal 2 terms (6 months) prior to your scheduled start date. If you are a new student planning to attend more than 6 months from now, your information will be active in the Patient Portal by April 9, 2012 or thereafter. At that time you will be able to log in and submit your immunization records and health history information. Thank you.

OSU Student Health Services uses an Electronic Health Records management system called eClinical Works. This system provides many features that help SHS provide efficient and high quality personalized healthcare to our patients. The Patient Portal is the patient's secure entry point to the system and his or her individual health record. Patients may also use the Patient Portal to contact their health care provider via a secure messaging system once logged in.

To Log-in to the Patient Portal:

• Go to https://bewell.oregonstate.edu
• User ID is your OSU Student ID number
• Password is your date of birth (mm/dd/yyyy format... INCLUDE THE SLASHES)
• You will be able to choose a new password after completing your first visit to the Web site.
